Drug Testing Consent and Policy Signature Forms
These forms were in your welcome envelope from Coach. If you need to print additional copies, they can be found on the Manheim Township Athletics page under their General Information tab. Return your signed forms to Coach.
Player/Parent Contact Information
Complete this form to provide your best contact information which will be added to TeamSnap, our streamlined method of communication that will be used throughout the season.
Purchase Game Day Socks
Purchasing socks will be the responsibility of each player. Suggested socks are Nike Over-the-Calf Soccer Socks in Navy and White, which can be purchased on Amazon, Nike or Dick's. Amazon link included above.
Yard Sign Order Form
Show your MT Pride with a yard sign. Complete this form to place your order. Orders are due 8/24.
Team Photo Order Form
Team photos will be taken on 8/27 at 2:45PM. Players should wear their blue uniform, including socks and cleats. Payment is due on photo day.
Spiritwear Store
Show your MT Boys Soccer Pride at games with an order from our Spirit Wear Store! Store closes 8/23. Orders will arrive in 2-3 weeks.
Varsity/JV Games - Ball Helper Sign Up
Freshman Team players contribute to the Varsity and JV Teams by serving as ball helpers for their games.
Each player should sign up for at least ONE game.
Concession Stand - Sign Up to Volunteer
Parent, your help is needed in our concession stand. Proceeds from our concession stand go directly to support both the Middle School and High School teams. Please consider volunteering to work in the concession stand on Senior Night 9/29.
This will allow our JV and Varsity parents to watch their players.

Please consider supporting the CKC and the MT Boys Soccer Program financially. Your support will assist in providing practice and training equipment, off-season and summer training programs, year-end banquet and awards, and a variety of additional team-related items that will support both the high school and middle school programs throughout the season.
